As the owner or authorized guardian of the dog(s) listed in the Intake Form, hereby authorize Cammino Canino to provide pet care services under the following terms and conditions. If I, my secondary contact person or my emergency contact cannot be reached in time:

I authorize Cammino Canino to seek emergency veterinary care for my dog if necessary for the dog's health and safety. In the event of an emergency, Cammino Canino is authorized to contact and consult with the veterinarian and make decisions on my behalf.

I authorize Cammino Canino to make decisions regarding the health and well-being of my dog and to seek veterinary care in the event of an emergency. This authorization includes but is not limited to, medical treatments, surgeries, diagnostic procedures, and the administration of medications.

I understand that I am responsible for any and all veterinary expenses incurred for the care and treatment of my dog.

I agree to reimburse Cammino Canino for any reasonable and documented expenses incurred for veterinary services and transportation costs.
