As the owner or authorized guardian of the dog(s) listed in the intake form, hereby authorize Cammino Canino to provide pet care services under the following terms and conditions.

I authorize Cammino Canino to let my dog off-leash.

I understand and acknowledge that off-lead activities involve some level of risk, including but not limited to the risk of small cuts and scratches, injury, accidents, encounters with other animals or people, as well as the potential for illness or exposure to bacteria and viruses.

I confirm that my dog has undergone basic recall training and will respond to verbal commands.

I understand that effective recall is essential for the safety of my dog during off-lead activities.

I hereby release, discharge, and hold harmless Cammino Canino, its owners, employees, agents, and representatives, from any claims, liabilities, damages, expenses (including legal fees), or harm to the dog arising from off-lead activities, except in cases of gross negligence or willful misconduct.
